Health Assessments

North Carolina General Statute § 130A‑440 requires that all students entering kindergarten, or enrolling in a North Carolina public school for the first time, submit a completed Health Assessment Transmittal Form within 30 calendar days of enrollment.

The health assessment must be completed no more than 12 months prior to the date of school entry. Failure to provide this documentation within the required timeframe may result in the student being excluded from school until the requirement is met.
