Diet Orders
Students who require modifications to school meals due to special nutritional needs or a diagnosed medical condition must have a completed Medical Statement for Students with Special Nutritional Needs for School Meals (Diet Order) on file. This form must be completed by a healthcare provider and returned to the School Nurse — please see forms below.
We cannot modify food textures, substitute foods, or otherwise alter a student’s diet without this documentation. Once received, our School Nutrition staff will review the Diet Order, adjust menus as needed, and guide cafeteria staff to safely support your child’s individual needs.
